We are a couple plus six children between the ages of 1.2 and 9.5. We are applying to you in a request to authorize assistance with nutrition for our baby, Similac level 3, as we are in great need.

My husband broke his ankle one and a half years ago, is currently using crutches and has never regained his health. He learns all day in the Nachalat Yitzchak Kollel but is not receiving a monthly stipend due to the financial difficulties of the Kollel. These days our income is limited to 840 NIS of child allowance and are waiting for answers from the National Insurance regarding disability and income guarantee to which we are entitled.

I cannot work due to health issues “fibromyalgia” (a type of rheumatoid arthritis) and hypothyroidism. I have a rheumatologist’s letter documenting my inability to work. I am home raising my children. Our baby is fed Similac level 3 , 2-3 times each day and we would like to continue and feed him the food he likes.

We would appreciate if you would accommodate our request and approve assistance for baby formula.
